Hartford Public Schools offers clinics this summer at no cost* for school entry physicals and immunizations at our School-Based Health Clinics. These services are available to Hartford residents in grades K to 12.

APPOINTMENTS ARE MANDATORY! Appointments for School Entry Physicals and Immunizations must be made by calling (860) 695-8780.
The clinics will be held at Weaver High School, Hartford Public High School, and MD Fox Elementary School on the following dates and times:
Tuesday, August 18: 10 am - 2 pm
Wednesday, August 19: 8 am - 2 pm
Friday, August 21: 8 am - 2 pm
Monday, August 24: 8 am - 2 pm
Once again, appointments are mandatory.
*All HPS' students are welcome, regardless of medical insurance coverage. Medicaid will be charged if the student is a Medicaid holder.
